Offsite Materials Request

Please note:

  1. You will not be contacted when your item(s) are available for pickup.
  2. You will be contacted ONLY in the event your item(s) cannot be retrieved.
  3. Any forms submitted after 1:00 pm will be processed the following work day.
  4. Items may be picked up from the circulation desk after 5:00 PM, Monday - Friday.
